Damage Observation Report Template
A damage observation report records the condition found at a particular place and time. Describe the visible evidence before discussing cause, responsibility or repair cost, which may require separate investigation and authority.

Define the observation and its limits
Identify the property, room, component and visit date. State whether you saw the condition directly or received information from another person. Note areas obscured or outside the agreed scope so the report does not imply a complete survey.
Use the appropriate urgent reporting process if the situation requires immediate attention. This template is for documenting observations; it does not replace emergency, insurance, incident or specialist assessment procedures.
A practical record structure
- Location: a repeatable property and component reference.
- Condition: what is visibly different, marked, missing or damaged.
- Extent: measured information where available, clearly distinguished from estimates.
- Evidence: context and detail photographs with captions.
- Source: direct observation or attributed supplied information.
- Next action: assessment, clarification or an authorised repair instruction.
Keep a reported event separate from the observed result. “Client reports impact during delivery” is a sourced statement; “delivery contractor caused damage” is a conclusion that requires an appropriate evidential basis.
Worked entry for a damaged panel
An illustrative entry is: “Reception desk, public face, lower panel beside entry end. Local indentation and surface mark visible at visit. Cause not established. Context photo locates the panel; detail photo shows the affected area. Coordinator asked to arrange assessment.”
If a baseline report exists, reference it and compare the same location. An earlier image may help establish that the area was visible at that time, but a missing or obscured view cannot prove the absence of previous damage.
Keep assessment and follow-up traceable
