How Do You Confirm a Verbal Conversation on Site?
Send a short factual summary of what you understood, identify what needs confirmation and keep it connected to the relevant item. Do not turn a suggestion overheard on site into an authorised instruction or treat silence as agreement unless an applicable process explicitly establishes that effect.

Record the discussion while it is fresh
Note the date, participants, item references and purpose of the conversation. Capture the practical outcome rather than attempting a dramatic transcript. If you did not hear part of the discussion, say what you need clarified rather than reconstructing it from assumptions.
Separate three categories: an observation, a proposal and an agreement made by someone with the relevant authority. “A replacement was discussed” is not the same as “replacement instructed.” If authority is uncertain, ask the project contact to confirm the appropriate route. A site note should not accidentally approve cost, scope or a variation outside your role.
Write a confirmation people can answer
An illustrative message is: “Following today's discussion at item D-09, my understanding is that the contractor will provide a proposed response before attendance is arranged. Replacement was discussed as an option, but no method was confirmed during our conversation. Please confirm the next action and who will issue any required instruction.”
Keep the message short enough for recipients to identify a misunderstanding. Use names or roles where needed, but avoid unnecessary personal details. Attach or link the exact item reference instead of sending a fresh unnumbered photograph that creates another thread detached from the report.
Make outstanding decisions explicit
- What proposal is awaiting a decision?
- Who can make or confirm that decision?
- What information is still needed?
- What work, if any, has actually been authorised through the proper route?
Where a target response date is agreed, record it as such. If you are merely requesting a date, label it as a request. Do not manufacture commitment by writing “as agreed” when the person only said they would check availability. This precision prevents a polite coordination message becoming a misleading account of the meeting.
Correct misunderstandings visibly
If someone replies with a correction, reconcile the point and circulate the clarified understanding to those affected. Preserve the sequence instead of editing your original note to make it appear everyone agreed from the start. Update the working action list once the decision is clear.
For urgent site concerns, follow the appropriate immediate communication procedure rather than relying on an email being read. A written confirmation supports that communication; it is not an emergency response system.
